I confirm that I am between the ages of 15 and 69 years old and that I have None of the under mentioned conditions.
- Pain in my chest when doing physical activity
- Loss of balance because of dizziness or loss of consciousness
- Bone or joint problems ( for example back, knee or hip) that could be made worse by a change in physical activity
- Currently not taking any medication for your blood pressure or heart condition
- I know of any other reason why I should not take part in physical activity.
If you are over 69 and are not used to being very active you should check with your doctor before you take part in the HypnoHIITS programme, clarify that it is safe for you to become physically active at this current time and in your current state of health.
Should you confirm the above, It is reasonably safe for you to participate in physical activity, gradually building up from your current ability level. A fitness appraisal can help determine your ability levels.

You acknowledge and agree that you have been previously assessed by a qualified medical professional (i.e. your doctor) who has given you consent to take part in physical activity.
